First Visit
Your first appointment will begin with a diagnostic evaluation in order to ascertain the nature of your problem. You will have time to ask Dr. Warren and Dr. Walker any questions you may have. After your evaluation we will discuss the necessary endodontic therapy and your alternatives.
Please assist us by providing the following information at the time of your consultation:
o Please make certain that your bring with you a list of all medications that you are currently taking.
o Your endodontic referral slip and any x-rays, if applicable. Please be aware that we may take our own digital x-rays to help in our diagnosis.
o If you have been advised by your physician or dentist to use antibiotic pre-medication because of mitral valve prolapse, heart murmur, hip or cardiac prosthesis, or if you have rheumatic heart disease please be sure to follow all pre-medication instruction prior to your visit.
o If you have dental insurance, please bring your insurance information. This will save time and allow us to process any claims.
o To save time you may register with us by filling out a Patient Registration Form, on our website please bring the completed form with you to your appointment.
Important: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Emergency Visit
It is common for patients to develop problems suddenly and we make every effort to see patients who are in pain as soon as we our office becomes aware of the problem. If you are in pain, please notify our office immediately, we will arrange for you to be seen as soon as possible.
